Having the Patient Intake Forms completed prior to your appointment will save you time in the office and are much easier to fill out at home ahead of time.

If you are unable to complete the intake forms on TherapyPortal before your appointment please arrive 20-30 minutes early to complete in our office. Intake forms must be completed prior to your appointment in order for your therapist to legally hold your session.

Please let us know ahead of time if you would like a printed copy of the forms ready for you to pick up at our office or mailed to you to complete at home prior to your first appointment.
