Action Phase

With the paperwork and first appointment completed, it is now time to put the work into counseling and therapy. Together, with their mental health professional, a person will create a mental health care plan, define goals, expectations and decide on the course of therapy.

Once important goals are identified and an action plan is put in place, evidenced based treatment approaches may be used to aid in reaching therapy goals. Treatment approaches are designed to increase understanding and change how a person thinks about themselves or a problem they may be facing.

Action Phase Steps

The Action Phase is where steps are taken to change behaviors and motivation is expressed to continue the journey towards improved mental health and wellness.

Disclosure & Relationship Building

In both counseling and psychotherapy, the relationship between a person and their provider is an important part of achieving genuine results. The therapeutic process relies on a strong foundation in order to succeed.

Our mental health professionals will work to understand and empathize with you, while providing helpful feedback that counters harmful behavior and prevents negative thoughts.

Identify the Need & Problem Assessment

For most people who seek professional help, at least one problem is initially identifiable. People often seek counseling following a certain breaking point, such as an angry outburst or panic attack. This can serve as a problem to be accounted for, and avoided in the future. Therapy aims to target mental disorders and cognitive differences that have turned into real-world problems.
  • Strained relationships
  • Poor communication
  • Intense feelings of sadness or hopelessness
  • Personal performance at work or school becoming difficult
  • Constant worry regarding relationships or circumstances
Although some of the problems listed above, may be remedied independently, it may be worthwhile to meet with a professional to determine whether the problem is one for concern.

Commitment to Action & Goal Setting

A person’s goals should be both aspirational and realistic. Although goals with mental health improvements are almost always aspirational, we must always choose realistic goals. It would be unrealistic to set a goal of feeling happy and fulfilled all of the time, because mood fluctuation is a normal experience.

During your initial sessions, a person will work with their mental health profession to determine short and long-term goals that will serve as milestones for progress.  Working as a team to develop a mental health care plan also known as a treatment plan is important because it is used to document goals, treatment options and acts as a map for the therapeutic process to provides a way of measuring whether therapy is working.

Motivation is the driving force that makes us want to achieve our goals in life. One of the tools that can be used to turn motivation into action is developing SMART goals. SMART goals are effective because they help us think through the details of what we are going to do so that we can put them into practice.

SMART goals are defined as Specific, Measurable, Attainable, Realistic and Time-sensitive plans of action. If, over time, progress is not made towards these goals, a discussion of alternate choices may be warranted and perhaps altering the frequency of treatment, methods of approach or referral to another provider may further progress.

Counseling Intervention

Counseling interventions will vary greatly between individuals. During the steps listed above, a mental health care plan will be developed. This plan is likely to include tasks that to will complete between sessions. During sessions, issues and problems will be explored through talking, which may also provide relief. In the event that the problems seem to revolve around a particular relationship, the other person may be brought into sessions if it seems that this would be helpful.

The plan may also contain basis information regarding the Treatment Approaches the provider may be implemented during your therapy.

Next Step 3 - The Decision Phase

Once a person feels confident about the skills and learnings they have received from therapy, it’s time to evaluate the effectiveness and decide to continue or end therapy. A person does not have to be completely symptom-free at the end of therapy, they should however, feel good about the support system set in place as well as more confident about responding to triggers and the hurdles they encounter in day-today life.
